Every client is required to fill out, read, and sign waiver forms, consenting to your tattoo or piercing.

You will be certifying that you have read and understood the risks associated with getting a tattoo or piercing.

We have given you the ability to complete these forms before your appointment.

If you have not completed these forms upon arrival to your appointment, you will be asked to complete them before getting your tattoo or piercing.

Please understand that we still require proof of identification at the time of your appointment. You must be able to provide a government-issued photo ID.

If you are under the age of 18 years, a parent or legal guardian needs to be present at the time of your appointment; Both parent or legal guardian and client will be asked to provide a photo ID and/or documentation proving that they are able to sign for the client or otherwise responsible for your care.

Anyone without a photo ID will not be pierced nor tattooed at Inkiadore, for legal reasons we cannot allow any exceptions.

By continuing with the booking process, you certify that you are over 18 or will be in the presence of a parent or legal guardian for your appointment.

MEDICAL CONCERNS:

If you have diabetes, epilepsy, hepatitis, hemophilia, HIV-AIDS, or any other communicable disease, heart condition or take medicine which thins the blood you must advise the tattooist and/ or body piercer.

If you have medical or skin conditions such as but not limited to, acne, scarring (Keloid), Eczema, psoriasis, rash, infection, lesion, freckles, moles or sunburn in the area to be tattooed that may interfere with the said tattoo, you must advise tattooist and/ or body piercer.

If you have any type of infection, rash, or lesion anywhere on my body, you must advise my tattooist and/or body piercer.
